The Nutrition Exam Worksheet follows a very similar path as the SOAP Notes module. Theory would state this is something great to automate, but practice indicates one would always need a back up system in the event of technical difficulties.
Option 1 - Paper
This paper form evolved over several iterations.
This enables a practitioner to make exam notes at the bottom, list the protocal and dosage on the top and work out bottle count and pricing from manual pricing sheets.
This worksheet can be staged in a sorting bar, scanned by date or by patient name (an either or unless 2 copies are made) and then recalled digitally as a pdf.

Option 2 - Nutritional Worksheet DB System
We have developed a digital version of this worksheet. It is very powerful for those that are interested in using it.
The functionality of this worksheet is similar in nature to the Point of Sale system which you'll see in a later section.

1 - Nutrition Worksheet
This is the same form but with digital inputs. This is linked to both the patient and the psops database. As a product is selected from the drop down the count per bottle will fill in. As quantities are specified the days/bottle will fill in allowing the practitioner to accurately recommend a bottle count based on revisit time frame. This worksheet will then also work up the total sale amount for the protocol. This is NOT intended to be used as a Point of Sale system. The products from this protocol can be easily cut and pasted into the Point of Sale System for checkout and they can also be pasted into the Product Description details worksheet for that print out for the patient if it is desired.
This is very advanced work for any database application, much less a spreadsheet. This Worksheet has a Hidden Header and it's not like any of the others we've seen so far.
Hidden Header section - Demo Data -- The top section enables the user/technologist to specify "demo data" for testing and training purposes.
Hidden Header Section - Equations -- The second section contains various equations that are used for math calculations, and by putting them here, they are out of reach of the user in normal work flow.
If you carefully look at the image above, just below the header there are three menu items on the document:
- Clear Form
- Get Demo Data
- Save Worksheet
These same menu items are outlined in RED in the image below with a 1 next to the red box.
- Clear Form -- The "Clear Form" link calls a java script routine which resets the form without saving the date
- Get Demo Data - The "Get Demo Data" link calls a java script routine which copies the demo data from the hidden header into the form
- Save Worksheet -- The "Save Worksheet" calls a java script routine which creates a unique ID for a primary key, it copies the worksheet header into a header record table and it copies the line items into a details table -- and with this form and this java script routine we have just created a relational database application with Google Sheets that operates very similarly to web based database applications. At the end of the routine, the worksheet will be "cleared", resetting it for the next patient

And of course, no system like this would be worth much if we couldn't reprint that original worksheet, right? Select the Patient from the list and their prior worksheets are listed in reverse date order.
This is all done with hidden header work and some hidden work in the _validationList tab. There is no java script associated with this recall.
